A ten dollar donation is requested for each appointment to help cover the cost of operating the clinic. All medical services are provide at no cost.  


 Patient Requirements: 

You must have a screening prior to seeing a physician.

Screenings are conducted on:

Monday-Thursday 10:00am-3:00pm

Wednesday evening 5:30pm- 8:00pm

If you do not speak English please bring a translator that is 18 years or older to translate in the screening.


What to bring to screening:


- Picture Id

- 2 Documents proving residency in one of the following zipcodes: 28269, 28216,28031, 28036, 28078, 28115, 28117.  This can be a Driver's License, Utility Bill (gas, water, power, phone) School Form/ District Letter, ect. 

- All proofs of household income

  • Salary (2 most recent consecutive weeks of pay stubs)
  • Letter from Employer Stating Wages (Letterhead and/or employer contact)
  • Current Tax Return or W2 for self employed
  • Unemployment Benefits
  • Statement Social Security/ITIN Card
  • Disability Benefits
  • Letter Workmen's Comp
  • VA Benefit Pension/ Retirement
  • Alimony
  • Child Support
  • Rental Income
  • All Other Income

Patients having living expenses provided for them must have their provider fill out and sign the following form to bring to the screening:

Patients who do not have any proofs of address (no ID or letters in the mail with the correct address) must complete the following form and bring it to the screening:

Patients who have no income must complete the following form and have a witness sign it. Then patients should bring this form to the screening.

Patients who do not recieve pay stubs from employer must have employer fill out the following form, then patient must bring the completed form to the screening.

In order to better fulfill our mission to improve the health of community we will begin offering health education opportunities. To sign up for one of these opportunities please call or visit our front desk

Weight management class: This class is designed to provide you with one on one counseling to help you loose weight. If you have a body mass index of over 30 you are at great risk for cancer along with many other health issues. If you have been looking for a way to help shed some pounds please sign up for this class. It will change you life!

Diabetes Education: If you or someone you love has been diagnosed with diabetes this class is for you. Learn more about the disease, what your diet should consist of, and learn about your options for keeping this disease under control.

Breast Education Classes: This class is designed to educate women on their risk of breast cancer and how to perform self breast checks. This class will be beneficial to women of all ages, but especially women over 50.
